2013 Folktoberfest NONAttendee Order Form

Mouse Purchase fees must be received no later than October 7, 2013. We will place our final order with the factory on that date. No mice will be ordered after October 7.

PAYMENT OPTIONS:

You may pay by check or PayPal (ID = nbr@charter.net) and you may make up to three installment payments. If you use PayPay, please add 3% to your purchase price to cover the fees they charge. If you wish to make installment payments, please note that on the final page of your order form. All mice must be prepaid no later than October 7. We will place our final order on that date.
